Interleukin 21 (IL-21) belongs to the IL-2 class of cytokines strongly linked to autoimmunity and inflammation. Moreover, it has divergent impacts on lymphomas and lymphoid cell leukemia. Clinical trials using IL-21 have provided favorable outcomes in cancer treatment, while the development of IL-21 inhibitors is ongoing for the treatment of autoimmune diseases.
The significant sources of IL-21 are activated CD4+ T cells, such as natural killer (NK) T cells and T follicular helper (Tfh) cells. IL-21’s biological functions are interposed through a heterodimeric receptor including the IL-21R and the common γ-chain (γc/CD132) receptor.
The latter is shared with other cytokines from the IL-2 family (IL‐4, IL‐7, IL‐9 and IL‐15). The IL-21/IL-21R signaling triggers the Janus kinases (JAK1/JAK3), and then the STATs (STAT1/STAT3/STAT5) signaling pathways, to control the differentiation, proliferation, survival and functions of myeloid, lymphoid and epithelial cells.
