One-stop news source keeps readers informed about key delivery model for health reform
Health2 Resources announced today free access to its Web portal, H2RMinutes, a one-stop source for news about the patient centered medical home (PCMH). H2RMinutes (http://www.h2rminutes.com) is designed specifically to keep health care policymakers, providers, employers and suppliers informed about the impact that the PCMH model is having on the national health care reform debate. The PCMH model is a fundamentally different approach to the way health care is delivered, putting the patient at the center of continuous, uninterrupted care that is managed and coordinated by a primary care provider. As Congress debates proposals to reform health care, the PCMH has emerged as a means to improve care delivery and lower costs.
The H2RMinutes Web portal supports a weekly free e-news brief, currently delivered to more than 18,000 national, regional and state PCMH stakeholders. Readers look to H2RMinutes to obtain a summary of timely and up-to-date news about community impact and national dialogue relating to the PCMH. H2RMinutes enjoys educational sponsorship from the Patient-Centered Primary Care Collaborative (PCPCC), a coalition of major employers, consumer groups, patient quality organizations, health plans, labor unions, hospitals, physicians and many others who have joined together to develop and advance the PCMH.
