HIRC recognizes CDPHP for offering best disease management program in upstate New York

Health Dialog Services Corporation, a leading provider of healthcare decision programs, today announced that its client, Capital District Physicians’ Health Plan, Inc. (CDPHP®), has been recognized by the Health Industries Research Cos. (HIRC) for offering the best disease management program in upstate New York.

This marks the fourth consecutive year that CDPHP has been honored by HIRC, a national, independent research organization serving the healthcare industry, as part of its Annual Report on the Disease Management Industry. Fewer than six percent of U.S. health plans profiled in the report are honored consecutively. This report, now in its ninth year, profiles leading disease management organizations (DMOs), assesses their growth trends, indicates their current partnerships, provides recommendations for future opportunities, and recognizes best current programs among health plans, employers, and vendors. It is based on extensive market projections and analysis, surveys, and in-depth interviews with healthcare industry executives.

“We celebrate the recognition of CDPHP as a leader in disease management,” said Jason Robart, Senior Vice President, Client Development, Health Dialog. “We are proud to work with CDPHP to deliver an effective program that encourages patients to take control of their decisions, make more informed choices, and live healthier lives.”

Health Dialog offers CDPHP members, a whole person health support program that includes disease management and decision support. Health Dialog provides sophisticated analytics, telephonic health coaching, and leading edge patient decision aids that help individuals make healthier choices and more informed decisions with their physicians on their course of care.
