Mediture, a leading software developer for
Program of All-Inclusive Care for the Elderly (PACE) programs, today
announced that they have implemented a web-based Electronic Health Record
(EHR) for more than 90 staff at Mercy LIFE's Adult Day Health Centers in
Philadelphia. Mediture assisted in the conversion from a legacy system of
patient demographics, medication lists and care plans and provided training
and on-site support during the implementation.
Mercy LIFE, a member of Mercy Health System, is committed to providing
medical and social support to the growing senior population in
Philadelphia. Founded in 1998, Mercy LIFE opened one of the first Program
of All-Inclusive Care for the Elderly (PACE) programs in the Philadelphia
Region. Today, Mercy LIFE serves over 300 seniors who are eligible for
nursing home placement, but are able to live independently in the community
with the support of Mercy LIFE staff. Mercy's interdisciplinary care team
provides medical and nursing care, therapeutic recreation, educational
programs, supportive counseling, meals, transportation and help with
personal care.
