John Walker, M.D., C.P.E., Chief Medical Officer, Cornerstone Health Care, is presenting Cornerstone's successful use of Humedica MinedShareTM Ambulatory as a basis for its new Patient Care Advocate program. Joining Dr. Walker in the presentation are John Cuddeback, M.D., Ph.D. Chief Medical Informatics Officer, Anceta and A.G. Breitenstein, J.D., M.P.H., Vice President & General Manager, Provider Markets, Humedica.

"Using the data to prioritize patients as part of a comprehensive patient-centered medical home program has turned out to be an outstanding means for identifying patients who require outreach and intervention," said Walker.

Dr. Cuddeback's presentation focuses on findings from Anceta's recent diabetes collaborative meeting. "We examined treatment variation amongst medical groups providing care to more than 200,000 diabetes patients and found significant differences in prescribing patterns and associated costs," said John Cuddeback. He added, "many groups are achieving similar improvements in glycemic control using the different treatment strategies with their diabetes patients. Guided by the data, our participants are learning from each other how to provide the best outcomes at the lowest overall cost."
