CAHF renews Preferred Provider deal with COMS Interactive

The California Association of Health Facilities (CAHF) has renewed its Preferred Provider relationship with COMS Interactive (COMS or Clinical Outcomes Management Solutions). With a goal of providing clinically-advanced products and services to the Association's member base, CAHF offers a limited number of opportunities to key industry partnerships.

The extension of the agreement with COMS expands the original three-year Partnership, executed in 2011, to focus on improving clinical and financial outcomes through advanced Disease Management. Since then, CAHF and COMS have enjoyed a strong relationship, with COMS actively participating in numerous CAHF activities and events throughout the year and CAHF members benefitting from improved outcomes.

"With the most recent CMS/Medicare guidelines regarding reducing readmissions and the anticipated decrease in reimbursement rates in 2009, it is incumbent upon CAHF management to identify to our members methods for improving care offered to their residents," remarked Jim Gomez, CAHF President/Chief Executive Officer. "The COMS flagship product, Daylight IQ™, serves to extend the nursing team's expertise, reduces hospital readmissions and improves the quality of care our members can provide to those in their charge, a winning outcome for all involved."

CAHF's membership is comprised of more than 1,300 licensed non-profit and proprietary health facilities. The Association's mission is to provide leadership, advocacy and education for long-term health-care professionals who serve the needs of California residents, and to promote integration of the healthcare continuum to ensure patient placement at the most appropriate and cost-effective level. With a focus on the consumer, CAHF serves as a catalyst and partner with other health care providers to promote and support excellence and integrity.

"CAHF is the leading voice for clinical and technical advancements in the post-acute environment, and with Daylight IQ™ their members continue to achieve significant benefits with documented improved outcomes," noted Edward J. Tromczynski, Chief Executive Officer, COMS Interactive. "We could not be more pleased to continue working with the forward-thinking CAHF management team and their members in our combined efforts to improve upon the healthcare provided to California residents."

The Daylight IQ™ online Care Guide Library presents up-to-date clinical information, signs, symptoms, and targeted outcomes. A configurable Nursing Assessment matches the dynamic Disease Profile for each resident, while a proprietary Rules Engine analyzes every resident's vital signs and clinical data points. Daylight IQ™ highlights abnormal findings and provides the nursing team with appropriate interventions and/or physician communication recommendations. The open architecture of Daylight IQ™ ensures easy integration with Electronic Medical Record (EMR) and other Skilled Nursing Facility applications.
