Alignment Healthcare, a company focused on improving the health of the Medicare population, announced that it has joined forces with HCA West Florida to better serve Medicare beneficiaries in three West Florida counties: Manatee, Pinellas and Sarasota.
Starting Jan. 1, patients will have access to Alignment Healthcare services at seven HCA West Florida hospitals in the region, including Blake Medical Center, Doctors Hospital of Sarasota, Englewood Community Hospital, Largo Medical Center, Northside Hospital, Palms of Pasadena Hospital, and St. Petersburg General Hospital.

"By accessing these Alignment programs for chronic disease management, pre or post hospitalization services, we hope to see outcomes and overall patient experience improve for members."
In January, Alignment is slated to open six care centers in Florida — four in St. Petersburg, Largo, Bradenton and Sarasota, and two in Jacksonville — to provide concierge-level care to Medicare eligible beneficiaries. The Sarasota center is located in HCA's Doctors Hospital of Sarasota.
Alignment ensures that all points of a patient's care are coordinated and integrated. Alignment Healthcare physicians, called extensivists, are a resource for the patient and primary care physician (PCP), seeing patients when their PCPs are unavailable at an Alignment Healthcare Center, the hospital or the patient's home, as needed.
Alignment Healthcare Centers, designed to meet the needs of Medicare beneficiaries, offer personalized and comprehensive disease management programs for chronic conditions such as diabetes, chronic obstructive pulmonary disease (COPD) and congestive heart failure, as well as wound care and pre-operative and post-hospitalization care.
