The “medical home” is a concept that, despite its name, is more of a perspective than a piece of property, and on Wednesday, February 24, Zurich, a leading property and casualty insurance provider globally and in North America, will host a webinar discussing the financial and performance-related implications of medical homes.
According to the American College of Physicians, “a Patient-Centered Medical Home is a team-based model of care led by a personal physician who provides continuous and coordinated care throughout a patient's lifetime to maximize health outcomes.”
Featuring an experienced panel of leading industry professionals, this webinar will highlight:
- How a hospital can function in a medical home structure;
- What the initial investment for a hospital-based medical home is likely to be;
- What elements of the medical home program promote cost-effectiveness and quality care;
- Real-life examples and lessons learned from those who have adopted the model; and
