HealthWyse, a leading provider of home health and hospice software and services, announced today that it has partnered with Cardiocom, a leader in home telehealth solutions, to seamlessly integrate patient information into HealthWyse's clinical software.

This software integration enables home health nurses and clinicians to have continuously updated information on patients suffering from chronic conditions including congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD), among others. Cardiocom's telemonitoring services track vital signs, symptoms data and other important information to help patients and their clinicians monitor their condition. By integrating this information into HealthWyse, home care clinicians have access to all of the patient's medical information at the point of care and in turn can make a positive impact on patient care.

HealthWyse customers are already seeing the benefits of the new software integration. The Norwell Visiting Nurse Association (NVNA) and Hospice, in Norwell, Mass., monitors nearly 130 patients using Cardiocom systems and immediately recognized the value of integrating Cardiocom data into HealthWyse. The data is available in real-time, providing nurses and clinicians in the field with a more complete patient record with which to make workflow and care decisions. As a result, caretakers can swiftly intervene in the event of an emergency, provide additional care when needed and deliver better patient care overall.
