A.O. Fox Hospital, a not-for-profit community hospital in Oneonta, N.Y. and affiliate of the Bassett Healthcare Network, has deployed Phytel's comprehensive, Web-based population health and patient engagement platform.
The hospital will use the Phytel technology to automate the identification of patients for recommended chronic and preventive care to improve quality and achieve better health outcomes. Phytel is also helping optimize the utilization of the hospital's employed primary care physicians (PCPs) by matching patient needs to clinical expertise and availability.

Fox Hospital selected the Phytel platform in part based on the positive experience and recommendation of clinical and business executives at Bassett Healthcare Network. Bassett has been using Phytel to help improve patient engagement in their own care since July 2014. A.O. Fox is affiliated with the Bassett Healthcare Network.
