There is a device called an Ommaya Reservoir that bypasses the BBB.  Five years of immunotherapy for my wife with existing drug, Herceptin mets into the brain/spine.  Forty plus additional women have had good results.  Short term this gets patients into treatment in weeks not years.  The transformed MAb with transferrin might be too specialized for non-primary brain cancers such as mets from lung, breast, etc.
