Health2 Resources announced today it has joined the Patient-Centered Primary Care Collaborative (PCPCC) as an Executive Member. Executive Members guide the PCPCC's efforts to advance an effective health system built on a strong foundation of primary care and the patient-centered medical home (PCMH).
Health2 Resources focuses on health care communications and analysis. The firm works with national clients, providing guidance and expertise on health care topics such as use of health information technology, new models of delivery, population health management, quality improvement and standards, case management, workplace wellness and health insurance exchanges. It works closely with community coalitions, think tanks and public policymakers on health care transformation initiatives, publishes research briefs and produces educational and networking events.

PCPCC's Executive Members represent a distinguished and diverse group of organizations inspired by a common vision: to transform the U.S. health care system. Members engage in PCPCC activities to advance innovation in care delivery, payment reform, patient engagement and benefit redesign. They work in partnership with PCPCC's general membership and staff to advance the organization's mission and strategic direction.

Health2 Resources is known for its widely read H2RMinutes; it launched in 2009 as the first news bureau service designed to keep policymakers, providers and decision-makers informed about health care transformation and the medical home model. Topics include the patient-centered medical home, accountable care models and the health insurance exchange marketplace.
